INFORMATION FOR NEW PATIENTS

 We see patients aged 18 and older only. We do not perform neuropsychological evaluations.

We wish that we could offer everyone a new patient intake, but our appointments are very limited. One of our physicians will contact you if they are able to see you; there is no need for you to call or email after completing this form. Please note that completing this form does not guarantee an appointment or a call back. We delete inquiries after 90 days whether or not we have been able to get you an appointment.

If we are out of network with your insurance company, we accept cash payment at the time of service and can provide a Superbill for reimbursement. Please make sure to check with your insurance company about the details of your mental health care coverage (ensuring the doctor is in network, deductible, co-pay, co-insurance, etc). We do not contact insurance companies for preauthorization or verification of benefits. 

For urgent psychiatric care, please contact your county mental health crisis center (Cascadia Behavioral Health in Multnomah County, OR and Hawthorn Clinic in Washington County, OR), your primary care physician or provider, go to an emergency room or Unity Center for Behavioral Health in Portland, OR, or call 911.
